Position Overview:

Successful candidate will have experience as a Hospital Liaison, preferably in Hospice Leadership. Role will be based out of Concord but will cover Alameda, Concord, Roseville, Sacramento, San Francisco, San Mateo, Santa Rosa, Saratoga, and Yuba.


Responsible for leading and overseeing the home health and hospice hospital liaison team to support seamless transitions from inpatient care to post-acute care services. Ensures that patient referrals, admissions, and discharges are coordinated efficiently and compassionately, while maintaining compliance with regulatory standards and organizational policies. Serves as a strategic partner to hospital leadership, physicians, and interdisciplinary teams, promoting Sutter Health as the provider of choice for post-acute care services.

Job Description:

EDUCATION:

  • Other: Graduate of an accredited school of nursing

CERTIFICATION & LICENSURE:

  • RN-Registered Nurse of California
  • BLS-Basic Life Support Healthcare Provider
  • AUTO-Automobile Insurance
  • DL-Valid Drivers License

TYPICAL EXPERIENCE:

  • 5 years recent relevant experience

SKILLS AND KNOWLEDGE:

•      Extensive experience in post-acute care.

•      Strong clinical knowledge of post-acute care.

•      Familiarity with reimbursement methods and regulatory requirements.

•      Excellent communication, leadership, and problem-solving skills.

•      Proficiency in EMR systems and documentation standards.

•      Ability to work in a fast-paced, emotionally demanding environment.

Pay Range is $168,958.40 to $270,337.60 / annual salary. Other Pay Range is $0.00 to $0.00 / annual salary. Outer Bay A105 Pay Range is $74.17 to $118.67 / annual salary.

The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
