The Home Health Nurse (RN/LPN) delivers high-quality, patient-centered care while coordinating with physicians, clinical managers, and interdisciplinary team members. This role is ideal for nurses who value autonomy, relationship-based care, and making a direct difference in patients’ lives.

Responsibilities

• Maintain timely communication with patients, families, physicians, clinical managers, and other healthcare team members regarding patient needs and changes in condition
• Participate in care coordination and discharge planning as appropriate
• Ensure professional standards are met in the provision, availability, and use of supplies, materials, and equipment required to safely implement the plan of care
• Prepare, submit, and maintain accurate documentation in accordance with company and facility requirements
• Complete visit and shift documentation on the same day services are provided

Qualifications

• High school diploma or GED required
• Current Nebraska RN or LPN license in good standing
• Ability to pass background checks (criminal history, APS/CPS)
• Strong communication and documentation skills
• Physically able to assist with transfers and lift 50+ pounds

Schedule

• Weekday hours
• On-call rotation with alternating weekends
