The Home Health Nurse (RN/LPN) delivers high-quality, patient-centered care while coordinating with physicians, clinical managers, and interdisciplinary team members. This role is ideal for nurses who value autonomy, relationship-based care, and making a direct difference in patients’ lives.
Responsibilities
- Maintain timely communication with patients, families, physicians, clinical managers, and other healthcare team members regarding patient needs and changes in condition
- Participate in care coordination and discharge planning as appropriate
- Ensure professional standards are met in the provision, availability, and use of supplies, materials, and equipment required to safely implement the plan of care
- Prepare, submit, and maintain accurate documentation in accordance with company and facility requirements
- Complete visit and shift documentation on the same day services are provided
Qualifications
- High school diploma or GED required
- Current Nebraska RN or LPN license in good standing
- Ability to pass background checks (criminal history, APS/CPS)
- Strong communication and documentation skills
- Physically able to assist with transfers and lift 50+ pounds
Schedule
- Weekday hours
- On-call rotation with alternating weekends
