Location: Tauranga | After-hours Outreach Service
Provide culturally safe, responsive health support to whānau through our HbU Health Service. This role works alongside our kaimahi and Prescribing Clinician to deliver clinical triage and outreach support, helping ensure whānau can access the care and services they need.
Working through a whānau-centred, community-based approach, you will support the delivery of flexible health and social support while helping raise awareness and participation in primary health initiatives across our community.
We operate a rotating roster; this position may include some after-hours mobile health service with flexibility to work around your schedule. The role is varied and hands-on, requiring initiative, efficiency and strong commitment to whānau-centred care and culturally safe practice
What You’ll Do:
- Provide clinical triage support within the HbU Health Service
- Work collaboratively with HbU kaimahi to support whānau wellbeing
- Provide administrative support to the Prescribing Clinician and follow-up patient recalls where required.
- Engage with whānau and community to promote health initiatives
- Support a culturally safe, responsive health service
What You’ll Bring:
- Registered Nurse with a current registration with the Nursing Council of New Zealand, or Enrolled Nurse, or could suit a similar Allied Health Professional such as a Health Care Assistant.
- Experience in community, primary care, health promotion or outreach health services.
- Strong relationship-building and communication skills
- Sound computer literacy MS Office. Medtech Evolution (or similar Patient Management System) experience desirable but not essential as training will be provided.
- Understanding of Te Ao Māori and kaupapa Māori approaches to health
