Public Health Nurse - Registered Nurse – Case Management

Join the Lorain County Health Department as a Public Health Nurse and make a meaningful impact in the lives of children with medical needs. In this full-time role, you will provide essential case management services while working under the general direction of the Community Health Nursing Supervisor to support the health and well-being of families throughout our community.



ESSENTIAL JOB DUTIES

  • Conduct comprehensive assessments to identify client needs and develop nursing diagnosis and individualized care plans.
  • Provide education and counseling on health topics, disease prevention, and self-management strategies tailored to the individual.
  • Coordinate and monitor health services, referrals, and resources to ensure continuity of care and optimal health outcomes.
  • Evaluate the effectiveness of the care plan and revise as needed.
  • Identify gaps or barriers in treatment plans and ensure clients receive appropriate resources and services.
  • Educate and empower clients to problem-solve and make informed decisions about their health.
  • Create accurate, concise and timely documentation in electronic health records. Maintain confidentiality in accordance with HIPAA regulations and organizational policies.
  • Develop effective working relationships with families, clients, community agencies, and other health professionals.
  • Demonstrate professional and supportive behavior. 

 

PREFERRED QUALIFICATIONS

Bachelor’s degree in Nursing with three (3) years of nursing experience preferred. Must have clear oral and written communication skills, Spanish speaking preferred. Competent in computer skills and electronic documentation.



REQUIRED QUALIFICATIONS

Must be a licensed registered nurse pursuant to Section 4723.13 of the Revised Code. Must have a valid Ohio driver's license with ongoing proof of auto insurance and be able to travel with reliable transportation. Current CPR certification. Must have proof of and maintain up-to-date vaccinations for healthcare providers or be in the process of completing. 



WORK SCHEDULE

This is a full-time (40 hours/5 days per week) position with usual hours M-F 8:00 am – 4:30 pm with occasional evenings and weekends.



Physical Requirements and Unusual Working Conditions: While primarily office based, this position requires travel throughout Lorain County, including client home visits. May encounter unsanitary working conditions. Must be able to wear PPE, including a face mask and face shield, when required. Must be able to lift objects of weight up to forty (40) pounds.



COMPENSATION: Benefits include medical, dental, vision, and life insurance. Ohio Public Employee’s Retirement System (OPERS). Paid vacation, personal day, holidays, and sick leave. Tuition reimbursement is available.



SALARY: $30.95 - $39.49/hr. Starting pay will be determined based on relevant experience, qualifications, and alignment with our established pay structure.
