Location: Lapalco Clinic – Harvey, LA
Schedule: Full-Time
Service Line: Primary Care with Nursing Home Rounding

About the Role

We are seeking an experienced, dedicated and compassionate Primary Care Family Nurse Practitioner to join our outpatient team. This role offers a unique opportunity to provide high-quality primary care in the clinic setting while also supporting geriatric patient care by providing medical services to residents in a local nursing home at least one day per week.

This position is ideal for a Nurse Practitioner who values continuity of care, enjoys building long-term patient relationships, and thrives in both clinic and post‑acute/long‑term care environments.

Key Responsibilities

  • Provide comprehensive primary care services to patients in an outpatient clinic, including physical exams, acute and chronic condition management, and preventative health services.
  • Perform health assessments and preventative screenings in alignment with physician guidance and established standards of care.
  • Develop and implement treatment plans based on patient needs, clinical guidelines, and collaborative decision-making with the supervising physician.
  • Evaluate patient responses to medical and nursing interventions and adjust care plans accordingly.
  • Provide at least one day per week of on-site primary care services to residents in a designated local nursing home.
  • Manage chronic disease conditions commonly seen in geriatric populations and ensure continuity of care between the clinic and nursing home settings.
  • Maintain thorough, accurate documentation in compliance with regulatory and organizational standards.
  • Collaborate with multidisciplinary teams—including physicians, nursing home staff, and ancillary providers—to promote high-quality, patient-centered care.
  • Provide patient and family education to support understanding, treatment adherence, and overall health improvement.

Qualifications

Required:

  • Master’s or Doctoral degree as a Family Nurse Practitioner.
  • Current Louisiana APRN licensure and certification.
  • Experience in primary care, family medicine, internal medicine, or geriatrics.
  • Strong clinical assessment and critical‑thinking skills.
  • Ability to work independently in both clinic and nursing home settings.
  • Basic Life Support (BLS) from American Heart Association

Preferred:

  • 2-3 years prior experience working in primary care,  long‑term care or skilled nursing facilities.
  • Familiarity with chronic disease management, geriatric syndromes, and transitional care needs.
