What You’ll Do
- Provide comprehensive primary care to patients of all ages, including preventive services, chronic disease management, and acute care
- Conduct thorough patient assessments and develop individualized treatment plans
- Prescribe medications and coordinate care with other healthcare professionals
- Educate patients and families on health promotion, disease prevention, and lifestyle changes
- Document patient encounters and outcomes using EMR systems
- Participate in quality improvement initiatives and stay current with clinical best practices
Qualifications
- Master’s or Doctorate degree in Nursing
- Active Nurse Practitioner license in Pennsylvania
- Board certification as a Family Nurse Practitioner (FNP)
- Experience in community health, outpatient care, or underserved populations preferred
- Proficiency in EMR systems and strong documentation skills
- BLS certification required; additional certifications (e.g., ACLS, PALS) are a plus
